Referral Process
We accept referrals for IBD patients from family physicians and specialists.
The referral is mandatory to set up your first visit to the IBD Clinic.
Your family physician or specialist will need to fill out the Consultation Request Form, attach all required investigations and fax the form to our IBD clinic. If you do not have a family physician, you can find one here.
Please be aware that we are not a walk-in clinic.
